Prescription Refill Request

Use this form only for:

  • a refill of a medication, preventive or prescription diet that has been prescribed within the past 12 months by an AMC veterinarian

  • the patient has been examined by an AMC veterinarian within the past 12 months

  • the patient has been stable on the medication - not worsening and no new diagnoses or major symptoms

 

For a new medication, a change in dose, a change in health status / diagnoses / symptoms that wasn't expected or a pet who is sick, please call us at 304-292-0126 or use the appointment request form.

 

We answer refill requests during business hours, Monday through Saturday. We will contact you if we have questions and when the prescription is ready for pick up. Check with us before heading over, as sometimes items can take a few days to fill if we have to special order the item(s).

Medication name, plus the strength and quantity if you know them. It is all on the label. List each item if there is more than one.

Larger volumes or whole bottles are generally less expenive pill for pill / bite for bite, and can save you a refill trip later. How much do you want?
